THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

MedmartRx, LLC ("We", "Our", "Us") is committed to protecting your health information. This Notice of Privacy Practices ("Notice") describes our legal duties and privacy practices and your rights regarding your protected health information ("PHI").

I. HOW WE MAY USE AND DISCLOSE YOUR PHI

• For Treatment: We may use your PHI to dispense prescription medications or to provide other health care services to you. We may disclose your PHI to other health care providers, such as your prescribing physician, to ensure you receive the proper care.
• For Payment: We may use and disclose your PHI so that the treatment and services you receive from us may be billed to and payment may be collected from you, an insurance company, or a third party.
• For Health Care Operations: We may use and disclose your PHI for our health care operations. These uses and disclosures are necessary to run our company and make sure that all of our patients receive quality care. For example, we may use PHI to review our treatment and services and to evaluate the performance of our staff in caring for you.

IV. CHANGES TO THIS NOTICE

We reserve the right to change this Notice. We reserve the right to make the revised or changed Notice effective for medical information we already have about you as well as any information we receive in the future. We will post a copy of the current Notice in the pharmacy with the effective date.

V. COMPLAINTS

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. To file a complaint with us, please contact our Privacy Officer at privacy@medmartrx.com. All complaints must be submitted in writing. You will not be penalized for filing a complaint.
