NOTICE OF PRIVACY PRACTICES

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

MedmartRx, LLC ("We", "Our", "Us") is committed to protecting your health information. This Notice of Privacy Practices ("Notice") describes our legal duties and privacy practices and your rights regarding your protected health information ("PHI").

I. HOW WE MAY USE AND DISCLOSE YOUR PHI

These uses and disclosures are necessary to run our company and make sure that all of our patients receive quality care. IV. CHANGES TO THIS NOTICE

We reserve the right to change this Notice. We reserve the right to make the revised or changed Notice effective for medical information we already have about you as well as any information we receive in the future. We will post a copy of the current Notice in the pharmacy with the effective date.

V. COMPLAINTS

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. To file a complaint with us, please contact our Privacy Officer at privacy@medmartrx.com. All complaints must be submitted in writing. You will not be penalized for filing a complaint.
